Completing the Forms
There are two forms that need to be completed before camp – we ask that you complete them at least two weeks prior to the start of your child’s camp session. You can fill out the forms by logging into your online account, or by downloading the forms and mailing them to us. If you would like us to mail you paper copies of the form, contact our office.
If any information changes after you complete the form, you can log in anytime to update the form, up to two weeks before camp. To make changes less than two weeks before camp, contact our office.
Which Forms Do I Need?
All campers need to have the following forms:
- Health History Form: Includes information about medication, dietary restrictions and allergies, immunizations, and medical conditions.
- Transportation Form: Inform us of your preferred check-in and pick-up location (Seattle, Tacoma, or at camp) and list authorized pick-up people.
- Medication Form: Complete this form if your child will be bringing medication to camp (including over-the-counter meds, prescriptions, and medicated creams/lotions).
Campers with special needs:
- Inclusion Assessment Form: For kids with special needs who need support from the Inclusion Staff at camp. If you are unsure whether to complete this form, contact our office.
